Adult Day Care
Planning and Administration
Two sites - Radcliff and Hodgenville
In-Home Respite to serve persons afflicted with Alzheimer's Disease (Hodgenville site only)
Assessment
An interview with a nurse or social worker is held to determine a client's situation, what assistance is needed and the best way to provide it.
Case Management
Case Managers develop Care Plans to provide services to older individuals while maintaining regular contact with agencies and clients to assure that services are provided and needs are being met.
Chore
Heavy housecleaning is done in order to maintain the person in his or her own home.
Disaster Assistance
Advocacy, outreach and training functions are held for the purpose of identifying older Kentuckians affected by disaster.
Escort
Personal transportation and accompaniment of individuals who have no other means of transportation to medical and other essential services.
Extended Respite
Twenty-four hour care provided for an individual for several days while the regular care giver is unable to provide this care.
Home-Delivered Meals
Meals provided in-home to frail older persons who would otherwise be unable to secure a nutritious meal. Meals are delivered hot or frozen depending on client's location. Diabetic meals are available with a physician's order.
Home Management
Household activities, including housekeeping, meal preparation, and shopping, are provided by a supervised aide in order to help maintain an individual at home.
Home Repair
Minor home adaptations, including additions to or modifications of the home environment to enable elderly citizens to maintain independent living in their homes, or to ensure health, safety or mobility. This service may include emergency response systems, safety inspections, and installation of smoke alarms.
Personal Care Attendant Program
Planning and Administration
Case Management and Assessment
Attendant services for persons 18 years of age and older who have lost the use of two or more limbs.
Personal Care Services
Non-medical personal care is provided to individuals which may include bathing, feeding, hair and skin care, and dressing.
Planning & Administration
Planning, administration and technical assistance for the development and delivery of services to meet the needs of the most frail and impaired elderly.
Respite Services
Care provided for an individual while the regular care giver is temporarily away from home.
Special Initiatives
Community Resource Fairs
Participation in area resource fairs in order to increase awareness of programs and services.
Lending Library
Books and video tapes on aging issues in caregiving, Alzheimer's Disease, medicare benefits, etc..
Lincoln Trail Service Providers Group
Coordinated interagency group involving area providers of human services.
Repair Affair
Coalition with Bank One and other agencies who volunteer to provide home repairs for low-income elderly homeowners.
Senior Celebration
Activities and workshops are held one day a year for senior citizens, sponsored by the Lincoln Trail Service Providers Organization
Senior Games
Regional activity highlighting older persons' physical accomplishments in competitive games.
